Tennessee Has Eleven of the Nation’s Underperforming Nursing Homes

The Senate revealed a secret national list of nursing homes facing issues, and it shows 11 of the worst performing nursing homes are located in Tennessee. The list indicates which nursing homes are on the Centers for Medicare & Medicaid Services list of candidates and those already part of the Special Focus Facilities program. While only 88 of the nation’s 15,700 nursing homes end up on the SFF list, an additional 435 are listed as potential candidates. The Special Focus Facilities Initiative gives operators with persistently poor care records incentives to improve their performance.

Special Focus nursing homes receive an inspection every six months compared with at least once every 15 months for all other facilities, including Special Focus potential candidates. Special Focus Facilities are also subject to civil monetary penalties and/or expulsion from Medicare and Medicaid eligibility if nursing home conditions do not improve.

Underperforming Nursing Homes in Tennessee

Eleven Tennessee facilities are included on the list, including several in Middle Tennessee. Facilities identified as ‘candidates’ for the program include:

  • Asbury Place at Maryville,
  • Bailey Park CLC,
  • GreenHills Health and Rehabilitation Center,
  • Creekside Center for Rehabilitation and Healing,
  • Rainbow Rehab and Healthcare,
  • Dyersburg Nursing and Rehabilitation Inc.,
  • Cornerstone Village,
  • Brookhaven Manor as.

Three other facilities are already in the program: Brookhaven Manor, Lauderdale Community Center, and Westmoreland Health and Rehabilitation Center in Knoxville.
